Thank you for choosing Quay Park Specialist Day Hospital to provide you with the care you require. We are committed to providing continued excellence in the health and wellbeing of people in our care, and we truly value your feedback.

If you wish to make a suggestion, raise concerns or make a complaint about any aspect of our service or facilities, we encourage you to speak to your Nurse, a Team Leader, our General Manager, or your Specialist. Alternatively, you can provide your feedback by filling out the form below or they can provide you with a feedback form if required.

Approximately one to two weeks after your hospital discharge, you will receive an email inviting you to complete an online patient experience survey. This survey is anonymous, unless you choose to be contacted and provide your contact details at the end of the survey. We review all the feedback we receive through this survey and use it to improve our services. We appreciate you taking the time to complete this.
